Why do you wish to volunteer at CUPS?


	Calgary Urban Project Society (CUPS) deals with individuals that may struggle with issues such as mental illness, addictions, abuse and poverty. I have been advised of the risks that can be associated with this population and understand to use caution and utilize the supports offered at this agency if there are any concerns. I hereby authorize Calgary Police Service, RCMP and Child Welfare authorities to release any information they may have regarding my past behavior to CUPS. I realize the Police Security Clearance is a necessary part of the screening process designed to protect clients of the agency and the integrity of the work the agency is involved in.
